Statement of Intent

New contractual requirements came into force from 1 April 2014 requiring that GP Practices should make available a statement of intent by 30th September 2014 in relation to the following IT developments;

Patient On-line Services

The practice offers patients the facility via Patient Access (App available) to:

  • Book, view, amend, cancel and print appointments online
  • Order online, view and print a list of their repeat prescriptions for drugs, medicines or applicances

The practice plans to offer all patients facilities to view online, export or print summary information from their record, relating to medications, allergies, adverse reactions and any other items agreed between the practice and individual patient, by March 2015 following configuration within our clinical system.

GP2GP

The practice makes use of GP2GP messaging technology, this enables us where possible, to receive their clinical records electronically as new patients join the practice. Similarly where patients leave the practice we are able to send their records electronically to their new practice (where enabled).

Summary Care Record (SCR)

The practice uploads for its patients a Summary Care Record on the basis of implied consent (unless you have opted out). A Summary Care Record is an electronic record, which contains information about the medicines you take, allergies you suffer from and any adverse reactions to medicines you have had.

The SCR should only be viewed in emergency or urgent care settings with the explicit consent of the patient unless the patient is unable to give their consent (for example a patient is unconscious).

Chadsfield Medical Practice confirms that your SCR is automatically updated on at least a daily basis to ensure that your information is up to date.
